Reference : Transaction done at your Vastral Ring Road, Ahmedabad Branch ATM and Net banking transaction number CWDR/000000008911/11-08-2017 18:07:39/SW
With above reference, would like to bring in your notice that, one month ago dt. 11/08/2017,i did a transaction at your Vastral Ring Road, Ahmedabad branch for Rs. 5000/- but cash and slip did not came our from the ATM machine (can be varified with ATM CCTV footage) and went to other ATM.
On 21/08/2017, i went to Kalol station branch for passbook, in that Rs. 5000/- is shown as debited from account dt. 11/08/2017. Immediately complaint was logged and a written complaint also logged on 22/08/2017 by self to kalol branch and waited for 10 days but no response received.
I called ATM customer care number 3 to 4 time regarding same but they informed to visit bank account branch.
I have visited Kalol branch many time after also but every time bank staff is coming excuse of " you will receive refund in two-three days" and avoiding me every time with bad behavior. Same is continued for 3 to 4 times and am tired now.

Requested to clear the issue on immediate basis and refund my Rs. 5,000/-.
Being a well known Govt. Bank, a early response is expected from your end at the earliest.
